138th Street – Grand Concourse is a local station on the IRT Jerome Avenue Line of the New York City Subway, located at the intersection of East 138th Street and the Grand Concourse in the Bronx neighborhood of Mott Haven. It has two side platforms and three tracks. It is the southernmost station on the IRT Jerome Avenue Line. The station, which has crossovers from one side platform to the other, also has mosaics indicating "Uptown Trains" and "Downtown Trains" in the mezzanine. It is served by the 4 train at all times except rush hours in peak direction and 5 train at all times except late nights.

The original name of the station was Mott Haven Avenue as indicated by the "MH" mosaic borders.

In 2011, the advocacy group Transportation Alternatives took a poll of subway riders to "rank the stank", or vote for the smelliest subway station in the system. The 138th Street – Grand Concourse station was voted the smelliest of four nominated stations by 35% of those voting. [2]
