Route 15 takes the following route through the State:
- North Providence: 3.4 miles (5.5 km); US 44 to Pawtucket city line
- Mineral Spring Avenue
- Pawtucket: 5.0 miles (8.0 km); North Providence town line to Massachusetts State line at Brook Street
- Mineral Spring Avenue, Main Street, [Church Street, Pine Street] (Main Street), Pine Street, Goff Avenue, Exchange Street, [Broadway, Cottage Street, Grove Street, Armistice Boulevard] (Exchange Street, Spring Street, Denver Street) and Armistice Boulevard.

Route 15 is a major east–west commercial corridor in North Providence, running along the old Mineral Spring Turnpike. In Pawtucket, Route 15 runs along the north side of downtown and through the Mixed-Up Mile.
Route 15 becomes an unsigned route, Brook Street in Seekonk, Massachusetts; it extends from the RI state line less than a quarter mile to a terminus at MA Route 152.
