Route 12 takes the following route through the State:
- Scituate: 8.3 miles (13.4 km); Route 14/Route 102 to Cranston city line
- Tunk Hill Road and Scituate Avenue
- Cranston: 8.8 miles (14.2 km); Scituate town line to Broad Street
- Scituate Avenue, Phenix Avenue and Park Avenue

Route 12 is a main east–west corridor through Cranston.
Route 12's eastern terminus is about 1/3 mile (1/2 km) past US 1A, at Broad Street. Broad Street is an old alignment of US 1A; it still carries several US 1A signs further south.
