I got most of the original list from tvheaven (including 3:13 and lost items) and corrected the capitalization. I got the dates for episodes 1 from the TVheaven images of Radio Times, and the rest from http://epguides.com/Doomwatch/. From http://epguides.com/Doomwatch/ I also capitalised My and added elipses in By the Pricking of My Thumbs. Can anyone confirm that no episode was transmitted on 8 August 1972?
