Apparently the RSR published a report in April or May of 2012, but it is difficult to find details. Here are some reports I found, but none are what I would call authoritative:
The page cites an article in the Newspaper Beeld published on 11 May 2012. Beeld is apparently an Afrikaans language paper. Someone familiar would need to look in the paper's archive and attempt to locate the article, and share a translated synopsis.

Argues that several points in the article are incorrect:
The speed at the time of the accident (234km/h), which the page claims is improbable
"The Beeld story says three passengers died in the accident. This is incorrect. One passenger died, and two crew members."
Says "The RSR finds that inadequate steps were taken by Transnet Freight rail personnel to secure the coaches when the electric locos were uncoupled, and that inadequately trained Rovos Rail staff were permitted to handle equipment without the supervision of competent persons. Presumably this refers to the vacuum brake system being released before the steam locomotive arrived. This delay was apparently due to points failure on account of a power problem."
