It appears to me that you used the file name from the hard drive of your computer, rather than the file name after the image was uploaded for use on Wikimedia projects. I took a guess about the most likely file name, and got lucky: the portrait now displays in the article.

This image has been published before on Mrs. Quinones Miller's website, so we need a confirmation that you are the original photographer and did not just copy it from that page. Please send an email as described above and also include a statement for File:Karen 2012b.jpg as this image has also been used elsewhere on the internet.
