The first confirmed case of the global outbreak of coronavirus disease 2019 (COVID-19) in the United States was announced on January 21, 2020. As of March 5, 2020, there were 159 confirmed and presumptive cases of COVID-19 in the U.S., including 11 deaths.[3]

As of January 31, 2020, the U.S. government requires Americans returning home after traveling in China's Hubei Province to submit to a 14-day quarantine. The United States is also denying entry to non-Americans who have traveled in China within the preceding two weeks. The US government has evacuated its employees as well as non-employee citizens from Hubei Province and from the Diamond Princess cruise ship in quarantine in Yokohama.

U.S. health authorities, including the Centers for Disease Control and Prevention, are urging local governments, businesses, and schools to develop plans like canceling mass gatherings or switching to teleworking so as to be ready when the need arises.[4][5] To prevent the spread of coronavirus, the CDC recommends washing hands often with soap and water for at least 20 seconds (or using an alcohol-based hand sanitizer with at least 60% alcohol if soap and water are not available), avoiding touching one's eyes, nose, and mouth, covering coughs and sneezes, staying home when one is sick, avoiding close contact with people who are sick, and cleaning and disinfecting "frequently touched objects and surfaces using a regular household cleaning spray or wipe."[6]

  • The CDC confirmed a second case in a woman in her 60s living in Chicago, Illinois.[85] The woman had recently traveled to Wuhan to take care of a sick parent.[86] On January 25, the CDC confirmed a third case in Orange County, California.[87] The person, a man in his 50s, was released from the hospital on February 1 in good condition to in-home isolation.[88]

  • The CDC confirmed the seventh case in a man in Santa Clara County, California, who had recently traveled to Wuhan.[98] The man recovered at home and was released from in-home isolation on February 20.[99] On the same day, the US Department of Health and Human Services declared a public health emergency, announcing a mandatory 14-day quarantine for US citizens who have visited Hubei Province within the preceding two weeks, as well as a complete denial of entry for non-US nationals who had traveled to China within the preceding two weeks. This was the first such order in more than 50 years.[100]

  • Six cases were confirmed among U.S. nationals in quarantine at Joint Base San Antonio-Lackland. One of the cases was a U.S. national evacuated on a flight chartered by the U.S. Department of State, while the other five cases had been evacuated from the cruise ship Diamond Princess.[121]

  • A case of unknown origin was confirmed in a resident of Solano County, California.[122][123] The California Department of Public Health said this may represent the first community transmission in the country.[124] The UC Davis Medical Center in Sacramento said that when the person was transferred there on February 19, the medical team suspected it was COVID-19 and asked the CDC to test for SARS-CoV-2. The CDC initially refused since the person did not meet the criteria for testing. The person was ultimately tested on February 23; the test results returned positive on February 26.[125]

On March 1:

  • The Rhode Island Department of Health announced a presumptive case in a person in their 40s who had traveled to Italy in mid-February,[137] and a second case, a teenager who had traveled with the first person.[138]
  • King County, Washington, officials confirmed two new cases, both men in their 60s. One was hospitalized in critical condition at the Virginia Mason Medical Center in Seattle, Washington; the other was hospitalized in "critical but stable" condition at the Valley Medical Center in Renton, Washington.[139] Those two cases were not linked to the cases in the Washington state nursing facility that were reported on February 29.[140][141] Later the same day, Washington officials reported four new cases associated with the LifeCare long-term care facility: a woman in her 80s, a woman in her 90s, a man aged 70s who had died, and a man in his 70s in critical condition.[142] Washington reported one additional case: a man in his 40s in critical condition,[12] bringing Washington state's total to seven new cases reported on this day.
  • Two cases were reported in Alameda County and Solano County, California, in health care workers at the NorthBay VacaValley Hospital. The workers were exposed to the patient in the case reported February 26 in Solano County.[143][144] In Santa Clara County, California, three more cases were reported, bringing the total number of cases in the county to seven. A fifth case was reported in a woman with chronic health conditions who is hospitalized; a sixth and seventh cases were reported in a couple who are both hospitalized, and who had recently traveled to Egypt.[145]
  • New York Governor Andrew Cuomo announced New York's first reported case of COVID-19: a woman in her late 30s, who apparently contracted the virus while traveling in Iran and who was isolating herself in her home in New York City.[146]
  • The second death from coronavirus in the U.S. was reported at EvergreenHealth in Kirkland, Washington.[147]
  • Florida became the third state that day to confirm its first coronavirus cases: one in Manatee County and one in Hillsborough County.[148]
  • Oregon confirmed its second case, a household contact of its first case.[142]

  • Officials in Washington state announced another four deaths in the state, bringing the U.S. death toll to six. They also announced four new infections in the state, bringing the state’s caseload to 18, and the country’s to 96.[149]
  • New Hampshire officials announced the state's first case, an employee with Dartmouth–Hitchcock Medical Center who had been to Italy.[150]
  • In California, the Santa Clara County Public Health Department reported two new cases, bringing the total to nine. Both were household contacts of previously confirmed cases and were in home isolation. [151] In Placer County, a healthcare worker who had close contact with a confirmed case in Solano County's NorthBay VacaValley Hospital was presumptively tested positive.[152] In Sonoma County, a resident was presumptively tested positive after returning from a cruise ship that departed San Francisco to Mexico.[55] In San Mateo County, an adult resident was presumptively tested positive; they were placed in isolation in a hospital. The source of exposure is unknown.[55]
  • Coronavirus cases in the country have reached 100, including repatriated citizens from Wuhan or the Diamond Princess cruise ship.[153]
  • Georgia officials announced the state's first two cases: a Fulton County man in his 50s who had recently returned from a work trip to Milan, Italy and his 15-year-old son.[154][155]

Governmental response

Federal

As the COVID-19 outbreak began, the federal government faced the crisis with a diminished capacity to respond.[14] After Donald Trump took office in January 2017, his administration had been slow to fill positions related to planning for and responding to pandemics. In April 2017, the Washington Post reported, "There is no permanent director at the CDC or at the U.S. Agency for International Development. At the Department of Health and Human Services, no one has been named to fill sub-Cabinet posts for health, global affairs, or preparedness and response."[202] The following year, "the Trump administration fired the government’s entire pandemic response chain of command, including the White House management infrastructure."[14][203] In late January 2020, Laurie Garrett, a Pulitzer Prize-winning science journalist, attempted to learn how the Trump administration was planning to deal with the COVID-19 outbreak. She reported: "In numerous phone calls and emails with key agencies across the U.S. government, the only consistent response I encountered was distressed confusion. If the United States still has a clear chain of command for pandemic response, the White House urgently needs to clarify what it is."[14]

On February 25, Dr. Nancy Messonnier, director of the Center for Disease Control and Prevention's National Center for Immunization and Respiratory Diseases, said at a news conference that "we expect we will see community spread [of the virus] in this country."[204] "We are asking the American public to prepare for the expectation that this might be bad ... Disruption to everyday life might be severe."[205] Larry Kudlow, the White House National Economic Council Director, said in an interview with CNBC that the United States has "contained" the coronavirus outbreak "pretty close to airtight."[206] Meanwhile, President Trump asserted that the coronavirus "is very much under control" in the United States, and that the "Stock Market starting to look very good".[207] Trump also stated told the media that he believes "the whole situation will start working out".[205] This came after Trump's comments earlier in February proposing a theory that the coronavirus "goes away in April with the heat". Messonnier herself had stated that it was "premature to assume that" theory would become true.[208]

On February 26, Trump insisted that the "risk to the American people remains very low", and that: "We're very, very ready for this, for anything".[209] Trump remarked: "I don't think it's inevitable" that the coronavirus would spread within the country, contradicting his own health officials.[210][211] He also criticized media outlets for making the "Caronavirus [sic] look as bad as possible, including panicking [financial] markets".[208]

Also on February 26, Vice-President Mike Pence was appointed by Trump to take charge of the nation's response to the virus.[212] The move received criticism from House Speaker Nancy Pelosi, who "expressed concern over Vice President Mike Pence's track record as Indiana governor on public health."[213] Pence's office issued instructions requiring senior officials to obtain approval from Pence's office before issuing any statements about the epidemic.[71] The administration explained this as ensuring messaging would be consistent, but many scientists and public health officials objected on the grounds that this would slow down the process of spreading accurate information.[71] The Wall Street Journal noted that "The directive follows Mr. Trump's frustration with messaging he felt was overly alarmist," and that it "came amid a sharp drop in the stock market in recent days on coronavirus fears, and after health officials had warned Americans to be prepared for a possible outbreak."[71]

As of February, the CDC was urging local governments, businesses, and schools to develop plans like canceling mass gatherings or switching to teleworking to be ready when the need arises. This includes having communication plans between communities, businesses, and employees, and planning for continued business operations in the face of increased absenteeism or disrupted supply chains.[5]

On February 27, reports emerged regarding a whistleblower report that alleged that United States Department of Health and Human Services "officials dispatched more than a dozen workers to aid quarantined Americans evacuated from China, without providing proper training or protective gear — a move that potentially exposed them to the coronavirus infection."[214][215] "The complaint alleges that teams from HHS' Administration for Children and Families were deployed to air force bases from 28–31 January and 2–7 February to receive Americans evacuated from Wuhan, China — the center of the virus outbreak — and that HHS officials broke protocol and ignored concerns about coronavirus exposure in the process."[215] As a result of the report, Senator Ed Markey called for the resignation of Health and Human Services Secretary Alex Azar.[215] A Solano County California Public Health Officer, Dr. Bela Matyas, stated "I have actually never worked with a more meticulous group of people...I mean, they were scary meticulous, and so the likelihood that that would happen is low to begin with. But it would be also no explanation whatsoever for this patient, because this is a random individual in a community of 450,000."[216][217]

On February 28, Trump spoke at a rally where he described the coronavirus as the "new hoax" of the Democrats. A day later, Trump said he actually meant that the "hoax" was Democrats blaming his administration for their response when "we've done such a good job".[218]

The United States would invoke the 1950 Defense Production Act to boost production of protective gear for SARS-CoV-2.[219]

The United States postponed a summit meeting with leaders of Southeast Asian countries, which was supposed to happen on March 14 in Las Vegas.[220]

US Department of Homeland Security offices in King County, Washington were closed for two weeks beginning on March 3 after an employee of the agency contracted coronavirus following a visit to the LifeCare nursing home in Kirkland, Washington.[221]

On March 2, Trump held a meeting with his federal response team and key members of pharmaceutical companies. Trump asked if "a solid flu vaccine" would also work as a coronavirus vaccine, and was told that it would not work. Trump also pressed for the companies to have a vaccine ready for the public within "a couple of months", but was told by Dr. Anthony Fauci (the leader of the National Institute of Allergy and Infectious Diseases) that it would take at least 1-1.5 years. After the meeting, Trump proceeded to tell the media that he heard that a vaccine would be available in possibly "a matter of months — and I've heard pretty much a year would be an outside number".[222]

Also on March 2, the Centers for Disease Control and Prevention removed from its website its count of the number of people tested for the coronavirus in the U.S.[223]

On March 4, Trump spoke on the Hannity program where he suggested that "hundreds of thousands of people" can recover from the coronavirus by "sitting around and even going to work". However, the CDC advises sick people to "not go to work, school, or public areas", while those displaying symptoms of the coronavirus should inform healthcare professionals.[224] Also that day, Trump blamed the Obama administration for supposedly instituting a rule which slowed down testing during the coronavirus outbreak.[225] According to experts, there was no such rule that was implemented by the Obama administration.[226][227] ProPublica reported on February 28 that the slow testing was due to the Centers for Disease Control and Prevention rejecting internationally-used World Health Organization COVID-19 test guidelines and instead trying to develop a test of its own; that test turned out to be unreliable, and most laboratories were not allowed to test until February 26.[228]

Congressional funding

On February 24, 2020, the Trump administration asked Congress for $2.5 billion in emergency funding to combat the outbreak.[229] On the same day, President Trump, while travelling on official business in India, tweeted that "The Coronavirus is very much under control in the USA. We are in contact with everyone and all relevant countries."[230] The Republican chairman of the Senate Appropriations Committee, Richard Shelby of Alabama, criticized the $2.5 billion as a "low ball" request.[231] Shortly after the Trump administration requested the $2.5 billion, Senate Minority Leader Chuck Schumer unveiled a plan for "$8.5 billion in emergency funding to combat the coronavirus."[232] Schumer's proposal was criticized by House Minority Leader Kevin McCarthy, who stated "Just picking a number out of a hat like that never seems to work properly. I would like to know what the experts believe they need, make sure we fund that."[213] As of February 26, McCarthy "said appropriators were looking at about $4 billion, saying he believes $2 billion is not enough."[213] A February 27 report, however, stated that Congressional "[l]awmakers are discussing a spending package that would provide between $6 billion to $8 billion to combat the coronavirus."[233]

By March 4, 2020, Congress had reached a bipartisan agreement for $8.3 billion in new funding to fight the coronavirus.[234] The deal included "more than $3 billion for the research and development of vaccines, therapeutics and diagnostics, as well as $2.2 billion for the Centers for Disease Control and Prevention, including $950 million to support state and local health agencies."[234] The $8.3 billion exceeds the new funding the Trump administration had requested, "with lawmakers on both sides of the aisle alarmed by what they deemed as the president’s paltry request to fight the bug amid criticism that his administration bungled its response and failed to keep the American people clued in about the potential severity of the outbreak."[234] The bill "passed the House overwhelmingly, with just two Republicans voting against it and 415 members supporting it."[235]

Travel and entry restrictions

In late January 2020, the Trump administration announced that effective February 2, quarantines would be issued for Americans returning from certain parts of China: "U.S. citizens who have been in China's Hubei province during the past 14 days and are returning to the U.S. States" are to "undergo health screenings and be monitored during mandatory quarantines of up to 14 days."[236] On February 16, 14 evacuees from the luxury cruise ship the Diamond Princess were flown on State Department-chartered planes from Japan to the United States.[237] The CDC "did not want 14 people who had tested positive for the new coronavirus to be flown back to the US, among hundreds of other uninfected people—but the CDC experts were overruled by officials at the US State Department."[237]

On February 28, CDC raised the South Korea,[238] Italy[239] and Iran[240] travel advisory to highest level. Travelers avoid all nonessential travel to the above countries and mainland China. Travel advisory to Japan has been raised to level 2, older adults and those with chronic medical conditions should consider postponing nonessential travel.[241] As of March 2, foreign nationals who have traveled to Iran within the last 14 days are denied permission to travel to the US.[242] US citizens and permanent residents returning to the United States who have traveled to Iran within the previous 14 days must enter through an approved airport.[242]

State and local

California

On February 10, Santa Clara County declared a local health emergency that would allow health authorities to deploy rapid response in the event of a potential coronavirus case. The state of emergency would be in effect for 30 days.[243] On February 14, San Diego County declared a local health emergency to ensure that the county had the resources needed to respond to the infections. The state of emergency lasted for seven days.[244]

On February 25, the mayor of San Francisco, London Breed, declared a state of emergency that would allow city officials to assemble resources and personnel to expedite emergency measures in the event of a potential coronavirus case in the city.[245][246] On February 26, Orange County declared a local health emergency to raise awareness and accelerate emergency planning.[246]

On February 27, Solano County declared a local health emergency to bolster response to COVID-19 cases.[247] California Governor Gavin Newsom announced that, as of February 27, the number of people being monitored for the virus in California amounted to 8,400.[248] The same day, "Newsom said the state was limited in how many people it could test because it only had 200 testing kits."[249]

On March 3, Placer County declare a public health emergency following the confirmation of a second coronavirus case in that county.[250]

On March 4, Governor Gavin Newsom declared a state of emergency after the first death in California attributable to coronavirus occurred in Placer County.[251][252][253]

Florida

On March 1, Governor Ron DeSantis declared a public health emergency after two cases were confirmed in Manatee County, Florida and Hillsborough County, Florida.[254]

Hawaii

Michigan

On February 3, the Michigan Department of Health and Human Services activated its Community Health Emergency Coordination Center to manage the state government's response to coronavirus.[256]

Oregon

On February 28, Governor Kate Brown created a coronavirus response team "tasked with coordinating state- and local-level preparations for an epidemic" of coronavirus in Oregon. "[C]omposed of directors or other representatives of 12 state agencies," the response team will "keep the governor posted on the coronavirus situation internationally and give her advice on how to protect the public."[257]

Texas

On March 2, San Antonio, Texas Mayor Ron Nirenberg and Bexar County, Texas both declared a "local state of disaster and a public health emergency" after an individual was mistakenly released from quarantine at Joint Base San Antonio-Lackland by the CDC before a third test for coronavirus returned a positive result.[258] The city subsequently petitioned the US government to extend the quarantine of US nationals at Joint Base San Antonio-Lackland; the petition was denied by Judge Xavier Rodriguez in the United States District Court for the Western District of Texas.[259][260]

Washington

On February 29, Governor Jay Inslee declared a state of emergency after the first US death attributable to coronavirus occurred in a man in his 50s with an underlying chronic health condition who had been admitted to EvergreenHealth Medical Center in Kirkland, Washington after complaining of severe breathing problems.[261] A second man in his 70s died at the same hospital a day later, the second US death attributable to coronavirus.[262] By March 2, the death toll in the Seattle area had risen to six.[263] By March 3, the death toll for the state had increased to nine.[264]

In a statement, Inslee said, "It is a sad day in our state as we learn that a Washingtonian has died from COVID-19. Our hearts go out to their family and friends. We will continue to work toward a day where no one dies from this virus."[265]

Researchers at the Fred Hutchinson Cancer Research Center and the University of Washington analyzed the genomes of the first reported case in Snohomish County, Washington, from January 20 and a more recent case on February 28, and determined the virus strain was related. Their findings indicate that the virus may have been spreading through the community for close to six weeks.[266]

Seattle's mayor Jenny Durkan has declared a civil emergency. [267]

Economic impact

Reactions

On February 21, 2020, Verizon pulled out of RSA Conference, joining the ranks of AT&T Cybersecurity and IBM.[268] On February 27, Facebook cancelled the F8 developer conference, which was supposed to occur on May 5 to 6 in San Jose.[269] On February 28, CBS shut down the production of The Amazing Race.[270] Multiple companies pulled out of Game Developers Conference including Amazon, Microsoft, Unity, Epic, Sony, Electronic Arts, Facebook, and Kojima Productions. As a result, the conference has been postponed for a later date.[271][272]

The annual March meeting of the American Physical Society, scheduled to start on March 2 in Denver, which is usually attended by 10,000 people, was canceled with less than 36 hours notice.[273] On March 2, 2020, Facebook announced that it would not be participating in the annual South by Southwest conferences scheduled to run from March 13 to 22 in Austin, Texas.[274]

Financial

Federal Reserve Chairman Jerome Powell announces a 50 basis point rate cut on March 3, 2020 in light of "evolving risks to economic activity" from the coronavirus

On February 27, 2020, the Dow Jones Industrial Average (DJIA) dropped 1,191 points, the largest point drop in history for the index, with a cause some attributed to anxiety over the escalating epidemic.[275] The same day, the S&P 500 logged a 4.4% decline.[276] The six business days it took for the S&P 500 Index to drop 10% (from February 20 to 27) "marked the quickest 10% decline from an all-time high in the index’s history."[276] Since the first case was announced on January 21, the DJIA has dropped over 3,500 points, accumulating to roughly a 13% decrease since the virus first arrived in the United States. This renders the market in correction territory.[277][278]

On March 3, 2020, the Federal Reserve lowered target interest rates from 1.75% to 1.25%,[279] the largest emergency rate cut since the 2008 global financial crisis,[280] in an attempt to counteract adverse effects of the 2020 coronavirus outbreak in the United States on its economy. "The coronavirus poses evolving risks to economic activity," the Federal Reserve said in a statement. "In light of these risks and in support of achieving its maximum employment and price stability goals, the Federal Open Market Committee decided today to lower the target range for the federal funds rate."[281]

Corporate revenue and earnings

In February 2020, American companies, including Apple Inc. and Microsoft, began lowering expectations for revenue because of supply chain disruptions in China caused by the virus.[282] In a February 27 note to clients, Goldman Sachs stated that it expects no earnings growth for U.S. companies in 2020 as a result of the virus, at a time when the consensus forecast of Wall Street expected "earnings to climb 7%."[283]

Cases

The Centers for Disease Control and Prevention (CDC) publishes official numbers every Monday, Wednesday and Friday, reporting several categories of cases: individual travelers, people who contracted the disease from other people within the United States, and repatriated citizens from crisis locations, such as Wuhan, China, where the disease originated, and the cruise ship Diamond Princess.[284]
