On the article USS CHICAGO SSN 721 , IT IS STATED THAT THIS IS THE FOURTH (4th) SHIP WITH THAT NAME. THAT IS NOT RIGHT. THERE ARE FIVE (5) OF THEM:

USS CHICAGO CG 11 MAY 1,1964 TO JAN 31,1984
USS CHICAGO SSN 721 OCT 13,1984 STILL SERVING THIS GREAT COUNTRY...

CA-136 and CG-11 are the same hull and considered the 3rd USS Chicago, so there have only been 4. See Chicago III, the US Navy's Dictionary of American Fighting ships, by the US Navy which discusses the 3rd USS Chicago CA 136 / CG 11.
