A tag has been placed on File:Pennsylvania Patient Safety Authority Logo.png requesting that it be speedily deleted from Wikipedia. This has been done under section F1 of the criteria for speedy deletion, because the image is an unused redundant copy (all pixels the same or scaled down) of an image in the same file format, which is on Wikipedia (not on Commons), and all inward links have been updated.

If you think this page should not be deleted for this reason, you may contest the nomination by visiting the page and clicking the button labelled "Click here to contest this speedy deletion". This will give you the opportunity to explain why you believe the page should not be deleted. However, be aware that once a page is tagged for speedy deletion, it may be removed without delay. Please do not remove the speedy deletion tag from the page yourself, but do not hesitate to add information in line with Wikipedia's policies and guidelines. Orphaned non-free media (File:The Pennsylvania Patient Safety Authority.png)[edit]

Thanks for uploading File:The Pennsylvania Patient Safety Authority.png. The media description page currently specifies that it is non-free and may only be used on Wikipedia under a claim of fair use. However, it is currently orphaned, meaning that it is not used in any articles on Wikipedia. If the media was previously in an article, please go to the article and see why it was removed. You may add it back if you think that will be useful. However, please note that media for which a replacement could be created are not acceptable for use on Wikipedia (see our policy for non-free media).

If you have uploaded other unlicensed media, please check whether they're used in any articles or not. You can find a list of 'file' pages you have edited by clicking on the "my contributions" link (it is located at the very top of any Wikipedia page when you are logged in), and then selecting "File" from the dropdown box. Note that all non-free media not used in any articles will be deleted after seven days, as described on criteria for speedy deletion. Thank you. Werieth (talk) 15:15, 1 October 2013 (UTC)[reply]

Speedy deletion nomination of Pennsylvania Patient Safety Authority[edit]

Hello Mminetti,

I wanted to let you know that I just tagged Pennsylvania Patient Safety Authority for deletion, because it seems to be a test. Did you know that the Wikipedia Sandbox is available for testing out edits?

If you feel that the article shouldn't be deleted and want more time to work on it, you can contest this deletion, but please don't remove the speedy deletion tag from the top.

You can leave a note on my talk page if you have questions. Op47 (talk) 16:56, 10 January 2014 (UTC)[reply]

If you mean do not delete your user page (which appears to be an article) then that is not my intention. However, Pennsylvania Patient Safety Authority needs to be deleted because it is obviously a test page and does not belong. When you have finished the article then you would be best off moving it to its correct place. The test article needs to be deleted for 2 other reasons, 1 it will prevent you from moving the proper article and 2 someone else may come along and write a proper article which would make it difficult for you. All in all, I would allow the article to be deleted and then as soon as you can, move your article into its place. Op47 (talk) 17:22, 10 January 2014 (UTC)[reply]

Help me![edit]

Help me with...

Pennsylvania Patient Safety Authority 18:26, 24 March 2014 (UTC)

I am unable to change my user name. Can you please assist me?

Hi, if you want to change your username, the easiest option for a user with only 380ish edits is to slap a {{retired}} template on your User:Mminetti account, abandon this account, then create a new account. Changing usernames isn't an easy process, but the steps are at WP:CHU/SIMPLE. Cyphoidbomb (talk) 19:32, 24 March 2014 (UTC)[reply]
UPDATE: After poking around your edits a little bit, I've noticed that you have already made a formal request to change your name to "Pennsylvania Patient Safety Authority". That's not likely to happen because that name represents a violation of our username policy. We don't want usernames that appear to be groups. So, you can change your name to something else if you want, a nickname or your real name, or whatever, but it can't be a group. Hope that helps. Cyphoidbomb (talk) 19:37, 24 March 2014 (UTC)[reply]

A tag has been placed on Pennsylvania Patient Safety Authority requesting that it be speedily deleted from Wikipedia. This has been done under section G12 of the criteria for speedy deletion, because the article or image appears to be a clear copyright infringement. This article or image appears to be a direct copy from http://patientsafetyauthority.org/PatientSafetyAuthority/Documents/Annual%20Report%202012%20.pdf. For legal reasons, we cannot accept copyrighted text or images borrowed from other web sites or printed material, and as a consequence, your addition will most likely be deleted. You may use external websites as a source of information, but not as a source of sentences. This part is crucial: say it in your own words. Wikipedia takes copyright violations very seriously and persistent violators will be blocked from editing.

If the external website or image belongs to you, and you want to allow Wikipedia to use the text or image — which means allowing other people to modify it — then you must verify that externally by one of the processes explained at Wikipedia:Donating copyrighted materials. If you are not the owner of the external website or image but have permission from that owner, see Wikipedia:Requesting copyright permission. You might want to look at Wikipedia's policies and guidelines for more details, or ask a question here.
