The silk glove-sign is a diagnostic measure when a hernia or hydrocele is suspected, however, there is no other clinical evidence to prove their existence. It involves rolling of the spermatic cord which causes a feeling of pieces of silk friction.[1]

Positive sign demonstrates thickening of the spermatic cord, representing the hernial sac, in comparison with that on the other side.

In fact, it is due to the thickening or silkiness of the cord as it crosses the pubic tubercle or, in the female, silkiness of the processus vaginalis over the pubic tubercle.[2][1]
