Bellini duct carcinoma (BDC, also known as low-grade collecting duct carcinoma[1] and tubulocystic carcinoma[1]) is a type of kidney cancer that originates in the duct of Bellini of the kidney. It is rare, accounting for 1-3% of all kidney cancers.[2] BDC progresses and spreads very quickly. Previously, BDC was commonly diagnosed as renal cell carcinoma. However, BDC does not respond well to chemotherapy drugs used for renal cell carcinoma. BDC may be related to urothelial carcinoma.


Discovery

BDC was thought to be renal cell carcinoma, until "recently developed techniques of lectin histochemistry" helped forward knowledge of kidney duct cancers.[3]

Study

BDC is extremely rare, and thus hard to study. In a well documented case, a 56 year old man was found to have BDC. In November 1994, he was brought in to the clinical study for an examination of his left renal mass. The patient was not suffering from flank pain or gross hematuria, common symptoms of urological disease. Through testing the patients disease was classified, and then underwent transperitoneal radical nephrectomy. His kidney was found to have a 43 mm (1.62 inches) tumor.[3] Currently, very few published works compare histopathological and clinical data. Histology

This cancer has a distinctive "bubble wrap" appearance.[1] It has been reported to occur together with urothelial carcinoma of the bladder,[5][6] and a cancer with intermediate histology also has been reported,[7] leading to the suspicion that these cancers are related.[6]

Symptoms

In a case with a 53 year old woman, the patient suffered from irregular fever, decreased weight loss and one episode of haematuria.[8] In her case, she suffered from the those symptoms, because the medication she was taking had no effect. In an extreme case, the patient died of BDC, after it metastasized in his lungs.[9]

Importance

In the conclusion of the case study mentioned above, the Nara Medical University of Japan, had this to say about BDC:

In conclusion, BDC is a rare tumor with a clinically rapid and progressively malignant course. This makes it important to determine the origin of all renal tumors and to distinguish rare forms such as BDC from common types of renal cell carcinoma. Moreover, it is highly desirable that a modality of treatment be established for this malignancy.[3]
