The Roman Catholic diocese of Hvar-Brač-Vis (Croatian: Hvarsko-bračko-viška biskupija; Latin: Dioecesis Pharensis (-Brazensis et Lissensis)) is a diocese in the Dalmatian islands in Croatia.

The diocese was founded in 1154 by the Venetians after their conquest of the island in 1147. The cathedral was set up at Stari Grad and later transferred to Hvar (city). This new diocese was subject to the Archbishop of Zadar who was under Venetian control already. This was the only way Venice could weaken the resistance of Dalmatia and its islands previously united under the strong Split diocese.^{[citation needed]}

In 1180, Hvar fell to King Bela III of Hungary and Croatia, who transferred it back to the archdiocese of Split in 1185. In 1185 the islands of Brač, Hvar, Vis, Lastovo and Korčula all belonged to the Hvar diocese now under the authority of Split.^{[citation needed]}

It is also known as the diocese of Lesina.
