The citizenshp of the Holy See is expressed by issueing the Vatican passport to that particular citizen. The ruler of the Vatican is the highest statesman of the Holy See (that is the Pope.
Any pope does get the citizenship of the Holy See by CIC (Canon law). Also, members of the Swiss Guards during their time of duty.
