A minority view held by some in the [[Republic of Macedonia]], and possibly elsewhere, is that Mihailov once treated the [[Macedonians (ethnic group)|Macedonian people]] as culturally distinct from the [[Bulgarians|Bulgarian people]] but had adopted a pro-Bulgarian position following the [[Second World War]], and indeed for the greater part of his life. Bulgarian historians however have consistently treated him as being ethnically Bulgarian and somewhat of a national hero and deny or tend to discount any statements he may have made referring to any such cultural distinctiveness.

In the Republic of Macedonia, Ivan Mihailov has been regarded as both a hero and traitor of the Macedonian people. This is most probably due to the fact that he expressed self-conflicting views in different points of his life.

However, some Macedonian historians have affirmed the typically Bulgarian view of Mihailov's ethnic Bulgarian consciousness.
