I wasn't sure if it belonged in the article, but Funston has a top 10 list of failures:
- failing to challenge assumptions
- lacking of vigilance
- failing to recognize velocity and momentum
- failing to identify relationships and manage complexity
- failing to anticipate failure
- failing to verify sources and corroborate information
- failing to maintain a margin of safety
- focusing on the short term
- failing to act on the right risks
