Episode Summary

Alex and Addison are acting awkwardly toward each other after their almost-kiss. Meanwhile, Mark seems to be upset with Addison, and she acts as though his anger is justified. Throughout the day, Addison seems to be having a lot on her mind, and when Callie pours her heart out to her about her feelings for George, Addison does not seem to be listening. Prompted by Callie, Addison tells her that she had an abortion after she became pregnant with Mark's baby, and that the baby's due date would have been that day.

Afterwards, Callie runs in to Mark, who realizes that Addison told Callie about the baby. When he tells her Addison told him that he would have made a terrible father, Callie asks him some questions, which makes Mark realize that Addison was right; that he would have made a terrible father.

Meanwhile, Heather, the girl with a curved spine as a result of VATER syndrome, is unable to undergo the operation that would enable to stand up straight, deemed by the insurance company as "too experimental." However, early on in the episode, she receives an anonymous $300,000 donation for her surgery. Bailey realizes that the anonymous donor is Izzie, asking her if she donated the money because she wanted to scrub in on the surgery. Izzie denies this, saying that Heather deserves this, and with that, Bailey tells her that she is once again becoming emotionally involved in a case and that therefore she could not scrub in.

While watching Derek and Callie operate on Heather from the gallery, George and Meredith receive pages and sprint off to George's dad's room. Noticing this, Callie asks to be excused from the surgery. Derek expresses his disbelief that she would want to miss this rare surgery that she had initiated; however, he lets her go, and Callie runs off to be with George.

Bailey is waiting for them, explaining that she paged because she needs to insert a new tube down Mr. O'Malley's throat so he can breathe, which can be a dangerous procedure with swelling from the surgery. Bailey seems anxious about performing the procedure, but she successfully inserts the new tube. Noticing that Bailey is visibly shaken, Chief Webber asks her if she is alright, to which she replies, "My son is named after his [Mr. O'Malley's] son. I need a minute."

Also in the hospital is Meredith's "extended family" after her half-sister delivers a baby girl, Laura, who is in the NICU. Although she's the intern on the case, her complicated relationship (or lack thereof) with her father, Thatcher (who, according to Christina and Izzie, has much in common with Meredith - which she flatly denies), compromises her work. When talking to the Chief, he says that there may be a side to her parent's story that she had never heard, which prompts her to ask Thatcher for his side of the story, or if he, too, snores - touching on the "things they have in common" conversation she had earlier with Christina and Izzie. He says that he does, and that wax earplugs help. Smiling to herself, Meredith leaves him with Laura.

Christina and Burke are still not talking in this episode, until Burke tells Derek that he hasn't experienced any tremors in a week, in front of Christina.

Following the surgery, Mr. O'Malley still has not regained consciousness. Overhearing his mother and Heather's mother talk about the risky surgeries performed on their loved ones, George realizes that the cancer must have spread, but the Chief and Bailey went ahead with the operation anyway. Hysterically, he storms in on the Chief and Bailey, that his father didn't know better when he asked them to proceed with the surgery no matter what, but they knew better. Furious, he walks away.

George then spends some time alone with his father. He tenderly wipes his father's forehead and face with a wet towel, speaking gently about a time when, as a kid, he didn't tell the truth, but his father believed him and took his side. George apologizes, and tells his father to fight just a little harder. The scene ends with him gently kissing his father on his forehead.

With Mr. O'Malley on life support, the O'Malley family discuss their options with the Chief and Bailey. The family turns to George for guidance, in sharp contrast with previous family scenes with his brothers and mother baby-ing him. George says, "He's not dad like this" and that it's time to let him go. They gather around Mr. O'Malley, holding his hands and shoulders in support while Bailey turns off the machine. Shortly after, Mr. O'Malley passes away.

In the hallway, George's friends are waiting for him in silence in the hallway as he walks out of the room. As he heads out of the hospital, Christina chases after him. She tells him that there is a "club" - a "dead dad's club" and "you can't be in it until you're in it". Christina reveals that her father died when she was nine, and that she was sorry that George had to join the club. When George responds by saying he doesn't know how to exist in a world where his dad doesn't, Christina simply says, "Yeah, that never really changes".
