Employer Health and Productivity Roadmap

The University of Pittsburgh Medical Center HealthPlan and WorkPartners programs developed an integrated strategy called the Employer Health and Productivity Roadmap, to address core drivers of poor health, excessive medical costs, and lost productivity. This framework consists of six main elements or milestones, with key metrics tied to each one. Each element and its associated metrics are to be reported quarterly to the employer to generate an action plan through the year.

Optimize environment

  • Improving the workplace environment by enhancing safety-related physical characteristics and practices, attitudes, behaviors, policies, and compensation schedules and promotion opportunities.
  • Conducting assessment "gap analysis" to identify key areas for potential improvement and to provide the basis for an action plan for the organization to move toward best practices.
  • Providng consultation, educational support, and skills acquisition through a "wellness committee" to initiate and sustain improvement efforts.

Increase healthy behaviors

  • Measuring and rewarding healthy "champion" employees.
  • Making health attainment a key leadership message.
  • Working to keep low-risk workers low and moving moderate and high-risk workers to lower risk.
  • Obtaining and tracking biometric measurements over time to further motivate behavior change and self-care.
  • Encouraging lifestyle coaching for employees.

Minimize avoidable or inefficient acute care

  • Improve healthy behaviors to reduce the prevalence of acute care and chronic conditions.
  • Communicating with providers through electronic health records, on-site or near-site clinics, and new virtual technology, to avoid possible inconvenience.
  • Expanding existing traditional occupational medicine services.
  • Using on-site health coaches.

Optimize chronic care

  • Becoming more actively involved in one's health care decisions for chronic disease.
  • Following competency-based "graduation criteria" for major chronic conditions, such as self-monitoring, adhering to prescribed medciations, and learning to take a more active role in decision-making with provider.

Reduce excessive surgery

  • Optimizing patient shared decision-making for low back pain and maximize behavior change and conservative therapies.
  • Considering full range of care options to avoid excess of "preference-sensitive" surgeries, and capital-intensive medicine.

Speed transitions from care to home and work

  • Monitoring the leading mental, physical, environmental, and medical conditions that drive time away from work.
  • Eliminating unnecessary, uncoordinated, and costly gaps in employee and family care or miscommunication among multiple provides.
  • Integrating primary, secondary, and tertiary prevention stratgies to better suit employee.
