Date Created in 1916 and issued in 1919. Date of photograph unknown.

Author Dora Ohlfsen is the original artist. The photographer is unknown; the photograph has been released as cc-by by Museums Victoria. It seems that galleries under their control own two of the medals and presumably they took this photograph.
