The headwaters of Peekskill Hollow Creek originate as the outflow of Lake Tibet in the town of Carmel in Putnam County, New York. It flows in a south westerly direction for approximately 17 miles (27 km) before joining Sprout Brook and then Annsville Creek.

The creek's drainage area is slightly more than 47 square miles (120 km2). The creek is dammed 0.8 miles (1.3 km) upstream of the mouth at Van Cortlandtville.
