Dr. Kelso signs the hospital up to RateYourDoc.org, a website where patients can evaluate their doctor's level of care online. Turk and Dr. Cox go to great lengths to get J.D. out of the running of becoming number one doctor on the website. They trick patients into believing the "The Todd" is J.D. so that they will bring down J.D's rating. J.D. can't believe that Turk went to these extremes. Dr. Cox and Turk talk to Dr. Kelso about their plan and get him to change J.D's rating back. Elliot brags on how awesome it is to really get to know patients since she diagnoses them over and over. One of Elliot's patients reveals that they intentionally overdosed on medication. Elliot decides not to tell her patient's at home nurse because she sees that even though it's wrong, she would rather be a good friend to her patient. Carla can't figure out what the Janitor's new girlfriend sees in him. Janitor disguises his crazy self to his girlfriend, and Carla tries to make him tell her the craziness that he has. When he does, his girlfriend is disgusted and Carla lies and tells that his craziness was all an act.
