Preliminary cleanup/expansion

I've done some merging/wikification here. Still needed is a discussion of friction vs. indexed shifting.

Here's an outline I wrote up a while back:

  1. Lead/intro
  2. Friction shifting
  3. Indexed shifting
    1. Twist shifters
    2. Trigger shifters - Shimano rapidfire
    3. Integration with brake levers - racing STI, Ergo; mountain Shimano dual-control on LX, XT, XTR
