Subject: [Surgery] Hi Judy – We [[are going to check on this and will let you know Thanks!Cathy]] Subject: Surgery Hi Catherine: Would you be [1] if the physician billed the "surgical global fee". I think it would be nice the for the member to have this information up front and it would earn some brownie points for us since the benefit is no longer available. I'll keep my fingers crossed ! Member's social security number is 016-60-3123.

Thank you. Senior Account Manager Employee Benefits Cc: Subject: Surgery Payment for services ends when the contract ends at midnight 2/28/08; services for the regular adjustments would not be covered. I also received the following information re: surgery/coverage/billing: If the provider bills the surgical global fee, that would include the surgery and post operative visits up to 90 days. If the member has post operative visits within that time where we have already paid a global fee the provider should not be billing for post operative visits separately as those visits are already included in that global fee package. If the services for additional post operative visits are after the 90 days and the coverage is cancelled, there are no additional benefits for payments on a cancelled policy. The member should discuss the post operative care with her physician. I hope that this information is helpful. Please let me know if you have any additional questions.

Thank you Cathy Subject: CDES: Surgery Hi, Got an issue that I hope you may shed some light - I have an employee Mary Smith (SSN to follow) who recently had surgery and requires additional treatments. Can you confirm if these treatments would be covered under the original surgery? The reason for our inquiry is that insurers plans were not designed to provide coverage for this surgery and these adjustment would not be covered. Essentially, we have a member caught in the middle. Kindly let me know what you find. Thank you. Account Manager Employee Benefits Confidentiality and Privacy Notice:This email contains confidential information.
