[edit]Given that one identity is questionable your call for a double check is reasonable. I found an online copy of a formulary we used back in the 1980s and I will check what is there. http://www3.mpifr-bonn.mpg.de/old_mpifr/imprs/bbl/bbl_downloads/jessner/nrlformulary.pdf I will do that in a couple of days.
