Paraplanaria
A simple narrow flatworm with two eyes. Illustrated guides indicate it's around ten millimeters long.

Paraplanaria is a genus of triclad belonging to the family Planariidae.[1][2]

Description

The genus is characterized by a "musculo-glandular posterior diverticulum of the atrium".[3]
