His paternal grandfather was the influential orator and senator Quintus Haterius. His maternal grandparents were the consul Lucius Domitius Ahenobarbus and Antonia Major. Antonia was a daughter of the Triumvir Mark Antony and Octavia Minor, sister of Emperor Augustus.

Antoninus was a first cousin of Empress Valeria Messalina, the consul Marcus Valerius Messala Corvinus, the consul Faustus Cornelius Sulla Felix and Emperor Nero and a second cousin to Emperor Caligula and Emperor Claudius. He was one of the two male descendants of Antony to carry a form of the name Antonius. The other was King Polemon II of Pontus, whose birth name was Marcus Antonius Polemon Pythodoros.

Antoninus was consul in 53. By 57-58, he had as a senator had squandered his inheritance through extravagances. According to Seneca the Younger, he was considered by some as a professional legacy hunter.
