The 2004 Dally M Awards were presented on Tuesday 7 September 2004 at the Sydney Town Hall in Sydney and broadcast on Fox Sports. Warren Smith presided as Master of Ceremonies.

Everyone in the NRL competition is eligible to be 'nominated' to win the Dally M Award. The winner of the award is decided by the most votes during the year as decided by a panel of judges on a 3-2-1 basis for each game played throughout the year.
