A paper printable Bitcoin wallet consisting of one bitcoin address for receiving and the corresponding private key for spending. Here on you can see another example of a paper printable Bitcoin wallet. It includes a link where you can see the actual transactions to and from that bitcoin address.

This image is public domain. I ran a contest on that resulted in the creation of this image in July 2012. I paid the awards offered in the contest, and one of the explicit conditions of participation and receiving the award was that the work be published in the public domain. The QR and address overlaid by is not copyrightable due to absence of creative content, as it consists only of an encoded random number, and the presence of these overlays does not change the public domain characteristic of this image in any way.
