As graduation nears for Seiji, Mayumi decides to make him some gloves as a present.(Which he confuses as great pot holders and is later seen using them for just that purpose.) He then takes her to his favorite spot where he used to run and together they enjoy his favorite view of the town. Seiji gives her a button off his uniform, commenting that he saved it just for her. Later that night Mayumi is seen sleeping in Sachiko's room. Sachiko’s ex-boyfriend, Suzuki, commits an atrocious crime with Mayumi as the tragic victim. Half-crazed and bent on revenge against the Sawada family, Suzuki mistakes the sleeping Mayumi for Sachiko and pours sulfuric acid all over her face. When Mrs. Sawada hears Mayumi's screams, she runs upstairs and is confronted by the crazed Suzuki brandishing a knife and demanding Sachiko's whereabouts. When she does not respond, he tackles her and slices her face with the knife. She merely erases the picture and takes her place in her seat. When her best friends walk in, they only hesitate for an instant before completely dismissing her; distancing themselves so not to be outcasts by association. During a track practice, Mayumi's friends are seen talking to this girl who questions them about the events that have occurred. She asks them if they were her best friends which they deny. Little do they know Mayumi overhears the conversation from behind the fence.

She inquires on what about his family's opinion and he replies that he will fix it and that he loves her. Sachiko says he can move in after Mayumi graduates. That evening Mayumi is in her room looking into a mirror, when she takes off her mask, her face is disfigured on the right side with a surgical scar still healing. In the mirror she sees the reflection of the lady in red (Kuchisake-Onna)and runs out of the room screaming to the phone to call Yuki. When she hangs up the phone, Mayumi goes to lock the front door, but the lady in red is seen standing outside. Mayumi then runs screaming into the kitchen. What looks to be the red lady approaching her is in fact Sachiko walking in from her talk with Kenji. The morning after, as the Sawada's are having breakfast, it is announced on TV that Mayumi's friends, Junko Muraoka and Kaoru Yamamoto, were attacked and killed by A lady in red. The next day, Mayumi goes to the funeral where she sees Seiji who came back for the funeral; after some talking she asks to go see the view with him before he leaves for Tokyo again. While she denies being close to Seiji and that, no, they never did meet up as they planned, Yuki believes she knows something but is not telling. Yuki asks for an early leave the next day to go home and search Mayumi's room, there she sees a doll that Mayumi had made in the image of Seiji. While looking around, Yuki comes across Mayumi's bloodstained dress from the day she was with Seiji before she killed him. Sachiko announces to Kenji that she believes she's pregnant. She asks if she should abort the child because of the bad circumstances surrounding her family which Kenji replies with a hard no. Later on, Yuki shows Sachiko the bloodstained dress and opts to go to the police; Sachiko says no saying that it would ruin what they had left. She asks for some time. The next morning she puts rat poison in Mayumi's juice. When she falls over and in moments ceases to move, Sachiko reaches over to check if she's dead-only to be cut by the scissors Mayumi had hidden behind her back before she died. Sachiko runs to get a bucket and rag and starts to clean up the blood while the distraught Yuki stares at Mayumi's dead body. To keep her from staring at Mayumi, Sachiko places a tan trench coat over the body. Throughout this whole episode, Sachiko convinces Yuki that it had to be done. That night they established an alibi as they drive to dump Mayumi's body. They dig a grave, put the body in (Tan coat and all), and fill it in. The next morning at work, Yuki is still distraught over Mayumi's death; unable to stop her hands from trembling. Sachiko finishes cleaning up the house. The climactic end scene shows a still alive Mayumi(or living dead one)walking back to find her sisters. As Yuki is cutting a customer's hair she sees Mayumi in the window. The owner greets Mayumi only to be stabbed in the gut by a very big pair of scissors (Note that she is wearing the now infamous tan coat). Yuki, who was already being eaten up with guilt, pleads with Mayumi and tells her she didn't know-only to have her fingers sliced off and to be killed by Mayumi stabbing her in the mouth (In the same style as Suzuki's knife in their mother's mouth).

Sachiko is at home unpacking her husband's stuff when Mayumi appears in the doorway. In a desperate attempt to escape Mayumi, she throws books at her only to be stabbed in the shoulder. Sachiko runs downstairs to the door, but decides to go to the kitchen and kill Mayumi for good. She stabs Mayumi with a large kitchen knife and she falls to the floor as Sachiko runs for help. Mayumi is not dead (or still undead but far from done) and runs after her sister. Sachiko sees people at the end of an alleyway and gradually makes her way towards them only to be stabbed repeatedly by Mayumi when she thinks she's finally safe. Before she dies Mayumi asks Kuchisake-Onna's infamous phrase "Am I...pretty?" There is no response and Sachiko drops dead. The movie ends with the people at the end of the alleyway not noticing what has happened. When it starts to rain, a little boy notices Mayumi dressed in the trench coat with giant scissors and Sachiko's dead body. As he stares in horror, Mayumi notices the boy and walks towards him, revealing the slit on the side of her mouth. At the end various voices can be heard talking about the legend of Kuchisake-Onna.

-"On May 18th, 1978, a serial killer claimed 13 lives and injured 52. Police found evidence linking them to murders of 2 other high school girls. A witness at Yoshizawa identified Mayumi Sawada as the suspect. A knife and large quantities of blood were discovered in the Sawada residence. Prints on the knife matched those of Sachiko Sawada, but not the blood type. The blood type matched Mayumi Sawada, but due to lack of advanced DNA testing, conclusive links were never made and the case remains unsolved. Seiji Moriyama's body was never found. Mayumi Sawada's whereabouts are still unknown...-"
