Plot

A farmer tends to shoot a boar, but misses his target. A flashback follows.

In Kochi, Ray Mathews is an up-and-coming pianist who fakes being blind to improve his piano skills. While crossing the street, he is knocked over by Anna. She takes care of Ray and soon they begin an intimate romantic relationship. Anna is impressed by Ray's talent and gets him an engagement at her father's diner. At the diner, a retired actor, Udaykumar, notices Ray and invites him to perform for his wedding anniversary. Ray arrives at Uday's flat and his wife Simi opens the door. Simi, convinced that Ray is blind, lets him play the piano. Ray sees Uday's dead body nearby but feigns ignorance and continues to play; he also sees Dinesh, Simi's paramour, hiding in the bathroom. Simi and Dinesh clean the body and stuff it into a suitcase while Ray plays.

Ray tries to report the murder to the police, but discovers that Dinesh is a police inspector. Meanwhile, Simi overhears her elderly neighbor, Mrs. Dicotta, talking to a police officer about Uday's murder. Simi later kills Mrs. Dicotta by pushing her off the ledge of their apartment. Ray sees the murder but is forced to continue feigning blindness. Later when Simi brings poisonous offerings for him after her husband's last rites and pulls out a gun, Ray admits to faking his blindness as an experiment to help his piano playing and living a cheap life. He says he will leave for London and will keep Simi's secret, but she drugs him.

A neighbor's child, suspicious of Ray's blindness, records a video of Ray fully able to see and shows it to Anna. As she arrives, Simi arranges things to look like she and Ray are together. Furious and heartbroken, Anna leaves Ray. When he wakes up, he realizes he has been blinded from the drug Simi gave him. Dinesh comes to Ray's home to kill him. Ray barely escapes, but he faints after hitting a telephone pole.

Ray wakes up in an illegal organ harvesting clinic. Dr Swamy and his assistants Lopez and Martha decide to spare Ray when Ray reveals he has information that will make them millions. They kidnap Simi, stage a suicide scene, and blackmail Dinesh. Lopez with the help of the other three receives the money at a construction site but Dinesh finds Lopez and shoots him. However, during the fight, Dinesh gets trapped under a free-falling elevator; crushing him to death while Lopez later dies from the after-effects of his injuries. The money is revealed to be counterfeit. Simi frees herself inside the clinic and attacks Dr Swamy when he enters; after a brief fight, he and Ray knock Simi out, tie her up in the boot of a car, and drive away. Dr Swamy reveals that Simi has a rare blood type and that her organs would sell for millions; he also plans to use her corneas to restore Ray's sight.

When Simi awakes in the car boot and begins making noise, Dr. Swamy stops the car to give her another sedation shot, but she overpowers him and seizes the wheel. Ray, thinking Dr. Swamy is still driving, tries to persuade him to release Simi. She drops Ray off and tries to run him over. A nearby farmer who is trying to kill a boar misses, causing it to jump and hit the windshield, which is where the story began. Simi loses control of the car and is killed.

Two years later at a gig in Oklahoma, Anna finds Ray, who still appears to be blind. After Ray tells her the whole story, Anna tells Ray that he should have accepted Dr. Swamy's offer to restore his sight as well as part of the reward money of one million dollars. Silently, Ray leaves and uses his cane to knock a can out of his path.

The success of Sriram Raghavan's Hindi film Andhadhun prompted regional film makers to remake the film in several Indian regional languages. The official announcement of the film being remade in Tamil as Andhagan,[1] in Telugu as Maestro,[2] and in Malayalam as Bhramam happened almost at the same time. AJ International signed actors Prithviraj Sukumaran, Mamta Mohandas, Raashi Khanna, Unni Mukundan and Shankar to do the roles of Ayushmann Khurrana, Tabu, Radhika Apte, Manav Vij and Anil Dhawan's roles respectively for the Malayalam version to be directed by cinematographer Ravi K. Chandran.[3][4]

Principal photography began on 27 January 2021 with all cast joining the schedule.[5][6][7][8] On 8 February, R. Rahul, an assistant director who was working on the film was found dead at his hotel room in Maradu in Kochi, Kerala. As per news reports, local police suspect suicide as he was found hanging.[9][10][11] Principal photography wrapped in early March.[12]
