My original request for this is [https://en.wikipedia.org/w/index.php?title=Wikipedia:Changing_username/Simple&diff=next&oldid=522251005 here]. It was explained to me as such:
I just had a look at your talk page revision history and the Matt Hannan user page revision history. It looks as though they never actually moved you at all – just moved the talk page. You created a page named "User:Matt Hannan" this month, when you added the WP:IR template, but there's no record of an account being created. On the other hand, the contributions page says that Matt Hannan performed all those edits. It's obviously a technical glitch. You need to bring this to Wikipedia talk:Changing username and see if somebody can sort it for you.
[[User:High Noonan|High Noonan]] ([[User talk:High Noonan|talk]]) 02:20, 25 July 2014 (UTC)

Changing username/Simple
This page is a forum in which users can request, and global renamers (including stewards) can consider and carry out, requests to rename accounts to usernames that are not yet taken on any Wikimedia project. While global renamers carry out renames, other users (including bots) help by "clerking" the mechanics of this process. If you wish to help as a clerk, please read the linked page carefully.

Instructions
For help 'vanishing' from Wikipedia,
please see courtesy vanishing.

Once you have decided on the name you would like:

  1. Read this page first. You must be aware of, and understand, all of that information before requesting a username change
  2. Check to make sure this username is not already in use. Do this by searching for the name here. If the name appears in the results, even if on another Wikimedia project, it is taken.
  3. Do not create an account under the new name or move/redirect your userpages. We'll do that for you.
  4. Log in with your present username in order to place your request. Renames will not be approved if the requester is not logged in.
  5. Click here to place your request.
    • Add your desired username after NEW=
    • Add a reason for the change after REASON=
    • Do not change anything after CURRENT=
  6. Make sure that you have spelt your desired name correctly, including capitalization and spacing. Save the page. If your request is submitted malformed, please remedy it according to this page.
  7. Check back on this page periodically to answer any questions that may arise regarding your request. Please be patient. It may take a few days for your request to be completed. If you do not answer any questions, your request may be declined due to no response.
  8. Once your request has been completed, please do not remove or change your request. It will be archived automatically by a bot.
  • Requests that do not adhere to these instructions or that are grossly malformed are subject to removal.

Important Notes After you are renamed...
  • Due to technical limitations, usernames may not start with a lowercase letter. However, you may use the template {{lowercase title}} on your user and user talk pages, and change your signature to reflect your preference.
  • Underscores and spaces are considered identical by the MediaWiki software.
  • Use of a company, organization, group, website, or product name as a username is prohibited by the username policy.
  • Please understand the implications of using your real name before requesting a name change to it.
  • ... your name will be changed to the new one in the page history of every page you've edited.
  • ... your userpage and any subpages will be moved to the new name automatically. Please do not attempt to move them yourself.
  • ... links left by your previous signatures will still point to the old userpage; however, a redirect will be left behind to your new userpage.
  • ... your request will be automatically moved to the archives in the interest of transparency.

Deutsche Schule Tokyo Yokohama → MaDSTY1

Robot clerk note: No problems found --Legobot (talk) 06:10, 17 July 2014 (UTC)[reply]
Bureaucrat note: @Deutsche Schule Tokyo Yokohama: An acronym is still promotional; however, if your requested name means "M at DSTY1", that is fine, but you will specifically need a name along the lines of "M at DSTY1" to avoid being blocked. Acalamari 08:31, 17 July 2014 (UTC)[reply]
Comment: perhaps he could just have it on his user page that it means M 'at' DSTY? It seems obvious that he knows that @ can't be in the username. MM (I did the who in the whatnow?) (I did it!) 21:47, 17 July 2014 (UTC)[reply]
I wasn't referring to the @/at sign at all. My concern is that some UAA reporters will still regard "MaDSTY1" as promotional enough to make a report, regardless of what a user page says (or whether or not they even read it); "M at DSTY1" or "MatDSTY1" are clear enough to prevent that from happening to this person. Androgirl101 → CarlaPetlyakov

Robot clerk note: No problems found --Legobot (talk) 12:40, 20 July 2014 (UTC)[reply]
Bureaucrat note: @Androgirl101: If you're worried about harassment, are you sure you want to change your username to your real name? (Assuming this is your real name, of course.) Acalamari 12:53, 20 July 2014 (UTC)[reply]

jaymo804 → cocobattles4567

Robot clerk note: The request was made by an IP (74.190.94.23 please login to request a name change. --Legobot (talk) 01:00, 21 July 2014 (UTC)[reply]
Clerk note: All requests must be made while logged in to the account that will be renamed. Please log in to the account that is to be renamed and confirm this request. Mlpearc (open channel) 02:44, 21 July 2014 (UTC)[reply]

CASIO F-91W → Callofduty4

Robot clerk note: There is already a global account registered for Callofduty4. --Legobot (talk) 11:00, 21 July 2014 (UTC)[reply]
Clerk note: Are you a Wikia Star and VSTF? Cool! But sadly, I'm afraid the global account might get in the way. You can try "TheCallofduty4" or something totally unrelated, and link to it from your Wikia userpage. And also, if you don't plan on editing here regularly (Wikia and Wikipedia are quite different indeed), I don't think having the same username on both sites really matters. --k6ka (talk | contribs) 12:41, 21 July 2014 (UTC)[reply]
Hey K6ka, thanks for your reply! If it's not possible to rename to "Callofduty4" then would a rename to "CallofDuty4" be possible? Doesn't seem to be an existing global account under that name. --callofduty4 (talk) 13:50, 22 July 2014 (UTC)[reply]
CASIO F-91W you could try es:WP:USURP to get "Callofduty4", they would probably grant the request. CallofDuty4 is fine as well, let us know. –xenotalk 14:03, 22 July 2014 (UTC)[reply]
I'll try there first. Thanks a lot for your help --callofduty4 (talk) 14:49, 22 July 2014 (UTC)[reply]
xeno - they declined it so... if CallofDuty4 is okay, I'll take that :P Sorry for the trouble --callofduty4 (talk) 10:54, 23 July 2014 (UTC)[reply]

Lineagroup → Linea Group

Robot clerk note: No problems found --Legobot (talk) 18:10, 21 July 2014 (UTC)[reply]
Clerk note: The username appears to be promotional. Use of a company, organization, group, website, or product name as a username is prohibited by the username policy. Please choose another name. Mlpearc (open channel) 18:50, 21 July 2014 (UTC)[reply]

Adnan Hz 97 → Hazbulator

Robot clerk note: No problems found --Legobot (talk) 04:30, 22 July 2014 (UTC)[reply]
Bureaucrat note: @Adnan Hz 97: According to this, not only do you have multiple accounts on other wikis but some of them have edits. Might I suggest that you make this request on Meta so that you can be globally renamed, which would rename all your accounts at once? Acalamari 08:21, 22 July 2014 (UTC)[reply]
Okay, i'm going to change it there and yes I have edits on other accounts. Thanks. Hazbulator (talk) 16:48, 22 July 2014 (UTC)[reply]
  • Not done happening elsewhere –xenotalk 14:44, 24 July 2014 (UTC)[reply]

Asheshneupane95 → Cricshady

Robot clerk note: No problems found --Legobot (talk) 13:50, 22 July 2014 (UTC)[reply]
@Asheshneupane95: perhaps you want to file at m:SR/UC because you have commons and wikidata accounts with edits. –xenotalk 14:06, 22 July 2014 (UTC)[reply]

babykish2020 → leo101

Robot clerk note: I have detected the following problems:
  • The requested username is already registered; see Special:Listusers. Please choose another name that does not appear on that list.
  • There is no global account registered for Leo101, however there are 6 unattached accounts on other wikis. --Legobot (talk) 12:00, 23 July 2014 (UTC)[reply]
Clerk note: The requested username is already registered; see Special:CentralAuth. Please choose another name which is not already in use. Mlpearc (open channel) 13:47, 23 July 2014 (UTC)[reply]

Dicksonwong → Dickson Wong

Robot clerk note: There is already a global account registered for Dickson Wong (primary wiki: zhwiki). --Legobot (talk) 07:20, 24 July 2014 (UTC)[reply]
Thanks for looking into this. It seems like the username diwo is not in use. It'd be great if the username can be changed to that instead. Thanks! --Dicksonwong (talk) 20:44, 24 July 2014 (UTC)[reply]

High Noonan → Matt Hannan

My original request for this is here.
