The Foundation of the Independent State of Croatia Memorial Medal (also known as Spomenznak na uspostavu Nezavisne Države Hrvatske) was a decoration awarded during World War II by the Independent State of Croatia.

The medal was established as a “visible sign of decoration for merits done, in peace or in war, for the Croatian people and the Independent State of Croatia. It was created by Croatian leader Ante Pavelić on March 18, 1942. The design of the medal featured elements that reflected Croatian culture heritage, including the trefoil ornament. In addition to the Foundation of the Independent State of Croatia Memorial Medal, the Independent State of Croatia awarded several other orders, decorations, and medals during its existence from 1941 to 1945.
