Pristimantis phoxocephalus is a species of frog in the family Craugastoridae. It is found in Ecuador and Peru. Its natural habitats are tropical moist montane forests, high-altitude shrubland, rural gardens, and heavily degraded former forest. It is threatened by habitat loss.
