Morcellation vs. Morcellator controversy

[edit]

Morcellation is not a new procedure in medicine. In 1893 there was a report in BMJ about myomectomy by morcellement (transvaginally), in 1899 John Lentaigne reported about a large cartilaginous tumour of thigh removed by morcellement. New is the use of electromechanical or bipolar (Olympus) morcellators. But not the instruments are the problem but the cutting into a tumour that can be done in every way.
One of the most recent studies compared laparoscopic hysterectomy with morcellation and abdominal hysterectomy. Their conclusion: "The risk of leiomyosarcoma morcellation is balanced by procedure-related complications that are associated with laparotomy, including death." (Matthew T. Siedhoff, Stephanie B. Wheeler, Sarah E. Rutstein, Elizabeth J. Geller, Kemi M. Doll, Jennifer M. Wu, Daniel L. Clarke-Pearson: Laparoscopic hysterectomy with morcellation vs abdominal hysterectomy for presumed fibroid tumors in premenopausal women: a decision analysis.
