Just follow the steps 1, 2 and 3 as shown and fill in the details

Thank you for contributing to Wikipedia. Remember that when adding content about health, please only use high-quality reliable sources as references. We typically use review articles, major textbooks and position statements of national or international organizations (There are several kinds of sources that discuss health: here is how the community classifies them and uses them). WP:MEDHOW walks you through editing step by step. A list of resources to help edit health content can be found here. The edit box has a built-in citation tool to easily format references based on the PMID or ISBN.

  1. While editing any article or a wikipage, on the top of the edit window you will see a toolbar which says "cite" click on it
  2. Then click on "templates",
  3. Choose the most appropriate template and fill in the details beside a magnifying glass followed by clicking said button,
  4. If the article is available in Pubmed Central, you have to add the pmc parameter manually -- click on "show additional fields" in the template and you will see the "pmc" field. Please add just the number and don't include "PMC".

We also provide style advice about the structure and content of medicine-related encyclopedia articles. The welcome page is another good place to learn about editing the encyclopedia. If you have any questions, please feel free to drop me a note. Doc James (talk · contribs · email) 23:46, 17 November 2018 (UTC)[reply]

You are not allowed to edit Wikipedia while the threats stand or the legal action is unresolved.
If you think there are good reasons for being unblocked, please read the guide to appealing blocks, then add the following text below the block notice on your talk page: {{unblock|reason=Your reason here ~~~~}}.  TonyBallioni (talk) 20:53, 18 November 2018 (UTC)[reply]
This user's unblock request has been reviewed by an administrator, who declined the request. Other administrators may also review this block, but should not override the decision without good reason (see the blocking policy).

Request reason:

I made legitimate edits to Richard Steadman's page but these were deleted and reversed inappropriately and then I noticed that every edit I made was reversed by DocJames and it is inappropriate for Wiki Editors to go round new users and carry out a witch huntTomEllis841 (talk) 23:58, 18 November 2018 (UTC)[reply]

Decline reason:

If you want to make any further unblock requests, please read the guide to appealing blocks first, then use the {{unblock}} template again. If you make too many unconvincing or disruptive unblock requests, you may be prevented from editing this page until your block has expired. Do not remove this unblock review while you are blocked.

  • You made threats of a legal nature involving off-wiki processes to another editor via email. As long as these are outstanding our policy does not allow you to edit. You have the option of letting the threats stand/acting upon them, which is governed by the jurisdiction where you live, or withdrawing the threats and letting us know that you do not intend to act on them. If you do the latter, any admin is free to unblock you. If you chose the former, as a private website we are allowed to say we do not wish you to be active here until the off-wiki process is resolved. I hope this clarifies the situation for you. The user in question uses Wikipedia as a reference despite being explained that this was not appropriate.
