The 2004 MTV Europe Music Awards took to Rome with Xzibit as the host. Held at Tor di Valle, it was the most expensive MTV Europe Music Awards to date and the first time the Colosseum had been used for this type of event. An external stage was erected and Anastacia performed "Left Outside Alone" to 400,000 Italian music fans.

The Free Your Mind award was given to human traffic charity, La Strada.

The mix of award presenters on the night included Jamelia, Alicia Keys, N.E.R.D, Naomi Campbell, Andre 3000, Kid Rock and Kanye West.

Eminem opened the show with "Like Toy Soldiers", Gwen Stefani took to the stage hanging from a giant clock to perform "What You Waiting For?" and Nelly teamed up with Pharrell Williams for "Flap Your Wings" and "Play It Off".
