Plot
Unlike previous seasons, the fourth season begins to shift into a darker tone. The season's core stories mainly revolves around Fiona's sudden trouble with the law, and Frank's possible death, in contrast to the beginning of the season, which revolved around Fiona getting past the disappearance of Jimmy and adjusting to her new job at Worldwide Cup. Making good money, she has begun dating her boss, Mike.
New developments have shifted within the characters. Lip no longer lives at the house, struggling to adjust to new life at college. Sheila works as a lonely maid to care for the kids, experimenting with online dating. Veronica is pregnant with triplets, as is her mother, also pregnant with a baby boy. Mickey is depressed over the disappearance of Ian, and has a strained relationship with his wife, Svetlana, who he has a son with. Mickey eventually tracks down and rekindles with Ian, who now works at a sketchy gay bar and has a new, odd behavior. Knowing that her husband is spending time with Ian, Svetlana begins to extort money from Mickey. In a partnership with Kevin, Mickey begins pimping Svetlana and her friends at The Alibi Room.
The police find Frank living in a drug house and return him home, to most of the family's dismay. Forced to stay on the wagon, Frank is desperate for a new liver, and snakes over to an apparent eldest daughter, Sammi, who lives in a trailer with her overweight son, Chuckie. Frank is warmly accepted into their home, though Sammi is furious when Frank accidentally slips that they're related; she eventually warms up to the idea of having a father, and cares for him. When Lip and Fiona kick the pair out for doing drugs, they break into Sheila's house while she's out of town. However, Frank's health soon begins to deteriorate; Sheila, wanting to adopt the kids from a new boyfriend, marries Frank to increase her chances of winning custody (Sheila ultimately fails to do so). While the season continuously dwells over Frank's possible passing, Frank survives after a last minute transplant when he gets moved up the waiting list.
Fiona's life is drastically turned upside down as the season progresses. Her boss, Mike, often has trust issues, and is devastated when he finds out that Fiona had hooked up with his brother, Robbie. That night, Fiona throws an impromptu party with Kevin and Veronica that involves alcohol and cocaine, which tragically ends when Liam gets into a stash of cocaine Fiona had been gifted from Robbie. Fiona is subsequently arrested, put on house arrest, and is convicted as a felon. Consequently, she fails to find a new job, and is eventually taken to a correctional facility for a 90-day sentence after breaking curfew.
The arrest of Fiona wrecks havoc on the rest of the family. Parental duties fall upon Lip, where he gets unexpected assistance from his roommate, Amanda, who he begins dating. He also puts his previous relationship with Mandy behind him; Mandy continues to be beaten by her abusive boyfriend, Kenyatta. Meanwhile, Ian's new behavior concerns the family. After going AWOL from the army, Ian shows signs of taking drugs and randomly falls into deep depressions; Fiona and Lip compare Ian's odd behavior to experiences with Monica's bipolar disorder. At Mickey's son's christening, Mickey publicly comes out as homosexual in The Alibi Room.
Being released early from the correctional facility due to overcrowding, Fiona is granted a job at a restaurant, Patsy's Pies, though she still has to attend anonymous drug meetings. As for Frank, he is up and back to drinking with his new liver, and has simultaneously brought Sammi and Chuckie into the Gallagher's lives. During the final episode, a post-credits scene shows Jimmy, who we haven't seen all season, watching the Gallagher house from his car with an unnamed woman.
