In June 2014, 20th Century Fox acquired the rights to Wan's graphic novel Malignant Man. According to reports, Wan was to develop the concept with a view to directing and producing the film. At the time of the announcement, comic book label BOOM! Studios was slated for a production role alongside co-producers Stephen Christy, Ross Richie and Adam Yoelin.[1][2] In June 2016, The Hollywood Reporter reported that Brad Peyton would direct the film.[3]

By August 2017, Rebecca Thomas was attached to direct instead of Peyton, and the title of the project was confirmed to be Malignant.[4] In July 2019, it was announced that James Wan would direct the film at New Line Cinema from a screenplay he wrote alongside Ingrid Bisu and serve as a producer alongside Michael Clear under his Atomic Monster banner.[5]
