Pristimantis medemi is a species of frog in the family Craugastoridae. It is endemic to Colombia. Its natural habitats are tropical moist lowland forests, moist montane forests, rivers, plantations, rural gardens, and heavily degraded former forest. It is threatened by habitat loss.
