We need a better photo of this plaque, please, we actual one is not clear at all. And why Ajax and Bayern webpages in internet did not speak about this plaque?

Obviously, that trophy has not awarded only to Juve.

About the alleged PSV plaque[edit]

Manchester United and Porto holds the record of appearances in Champions League since the introduction of the current format with 15 (cf. UEFA Champions League 2010/2011 Introduction — Records: UEFA Champions League competition records, p.4) and and United hold the record of consecutive appearances (14 since 1996-97 season without consider the current season). As PSV have 13 and 12 appearances, respectively, "their" UEFA plaque does not correspond to a record in the tournament, but the simple act of participating as surely have been the United and Porto (the record holders). So, the award assigned to PSV is not a relevant sporting recognition. Also, PSV have received a UEFA plaque by their appearance in the 2009-10 Europa League (first time since 2007-08 season).--Dantetheperuvian (talk) 20:02, 10 November 2010 (UTC)[reply]

You're right, the insertion of this plaque was nonsense. The title of the reference says it all: "PSV receive UEFA plaque for the thirteenth time". And then: "UEFA delegate Pierino Lardi presented PSV the traditional plaque, which every participating club receives, ahead of the first home game." This is not a problem of relevance or records. The title of the page is "The UEFA plaque", not "records linked to a UEFA plaque". If the topic of this page is (are) the UEFA plaque(s), it's completely POV to choose a single plaque as the sole topic of this page. If PSV received a UEFA plaque (and this fact is referenced), they must appear in this page. If we find other clubs which received a UEFA plaque, they will appear in this page. --79.7.142.132 (talk) 15:15, 12 November 2010 (UTC)[reply]
This article is not about any plaque(s) UEFA presented to some club some time. That would be a long useless list. It is about the specific plaque Juventus received for being the first side to win the three major UEFA trophies.
