Pathophysiology[edit]

RHD viruses target hepatocytes, cells of the mononuclear phagocyte system, and cells of the monocyte macrophage system. Death is due to liver failure or disseminated intravascular coagulation.[1]

Both viral strains of RHDV replicate in the liver, causing hepatic necrosis and liver failure, which can in turn lead to , hepatic encephalopathy, and nephrosis.[2] Bleeding may occur, as clotting factors and platelets are used up.

histopathology RHD causes apoptosis and variable lytic and coagulative hepatocellular necrosis with a periportal, multifocal, or diffuse distribution. The necrosis can be accompanied by variable hemorrhage, congestion, and sinusoidal fibrin thrombi. Inflammatory infiltrates consisting of heterophils or macrophages are generally mild and noted only in less acute cases.[1]

Fibrin thrombi can occur throughout the body, and are most often found in the lungs, glomeruli, and spleen. The spleen can also show lysis of lymphocytes. Hemorrhage can be found in multiple organs.[1]
