I figured we should move this discussion here, so we can stop pestering Ritchie with it. Anyway, I think I've figured out what happened this table. The 1968-69 ICFC was the first to have teams qualify based league position. Entrants to the previous cups where I think chosen arbitrarily by the organizers. If you look at previous Football League tables (66-67 for example), the table denotes participation in that year's ICFC rather than qualification to the next one. You're right about the qualification criteria for the ICFC, it changed in 1968 (presumably starting with the 1968-69 tournament). Before then it was open to clubs or 'Representative Teams' from cities that had held trade fairs only, and not on final league positions. Very strange.
