I would like to request an unblock and change of user name. Please let me know what steps if any I need to take.

While patrolling new pages I found the one you created, Sioux falls seminary. First of all, as this appears to be your first edit, welcome to Wikipedia. I hope you'll stay. Be advised that since your username is Sfseminary, someone will likely challenge your article and try to delete it. It looks as if you are advertising. Though your article isn't a bad start and the seminary is notable, do not be discouraged if it is deleted. I think this article should stand.
