Also removed the Burnham, IL (1987) incident as there was no failure of a derail device in that accident. You can find proof of that on page 86 (102 in the pdf) of the following NTSB railroad accident report brief: https://www.ntsb.gov/investigations/AccidentReports/Reports/RAB8902.pdf where it states: "NO DERAIL DEVICE ON TRACK" at the end of the "Narrative."
