Plot
Fiona adapts to life with Jimmy, as he officially moves in with the Gallaghers. Jimmy, however, has more pressing concerns, his life on the line as he continues being forced to act as a faux husband to Estefania, which Fiona is oblivious to.
Frank calls Child Protective Services on his own family after being kicked out of his own house once again. When a CPS worker comes for a visitation at a particularly hectic time, the underage kids are forced into separate foster homes, many being less-than-adequate. Though Veronica and Kevin attempt to foster the kids, Fiona ultimately gets Frank to clean up for their foster worker, convincing Sheila to play Monica. In court, Fiona fights for custody of the children against Frank, and is declared their legal guardian. This is bad news for Jimmy, who had plans to go to medical school in Michigan. Though Fiona is shocked, she's supportive, until she finds out that Jimmy had applied for a studio apartment only. After Jimmy lashes out at Fiona during an argument over the apartment, Fiona ends their relationship.
Unlike the previous season, Mickey and Ian have a greater focus in the third season. With their relationship continuing to grow, all their progress comes to a halt when Mickey's homophobic father, Terry Milkovich, unexpectedly walks in on them. Terry beats them both at gunpoint and forces a prostitute, Svetlana, to have sex with Mickey. Mickey is thereby damaged and the two drift apart; Ian soon learns that Mickey is marrying Svetlana, to his disdain. Mandy and Lip are together, though Lip continues to hold a torch for his ex, Karen, who has not returned after her fall out with Sheila. Worried that Lip is throwing away his future, Mandy secretly applies Lip for many colleges, and a successful interview gives Lip a chance at MIT. Meanwhile, Kev and Veronica continue their quest to have a baby, but there is little chance of Veronica getting pregnant; Veronica asks her mother to be her surrogate.
Family troubles are not limited to the Gallaghers: Sheila, still dating Jody, wants to find out where her relationship with Karen faltered, and where Karen's loose behavior started. After a short hiatus, Karen meddles back into town mid-season, though the reasoning for her return is fairly egregious. Scheming against her mother, Karen had called the Wong family to take Baby Hymie away from Sheila and Jody, further straining their mother-daughter relationship. Lip is also affected by Karen's return, as it conflicts his relationship with Mandy; Mandy vengefully runs over Karen in the middle of the street, leaving Karen with possibly permanent deficiencies. The accident had interrupted Sheila's heartfelt apology to her daughter, which was cut short moments before the incident happened. Lip angrily breaks things off with Mandy. To give Karen a better recovery, Jody leaves with Karen and Baby Hymie to Arizona, saying goodbye to Sheila.
The final episodes, while still comedic, shift into a much darker tone. Frank is diagnosed with liver failure and is advised to stop drinking. When he fails to do so, he is sent back to the hospital with his life on the line; Fiona tries convincing him to stop drinking for either himself or his kids, but he refuses. Estefania is deported due to Jimmy's ignorance; Estefania's father orders Jimmy to board a yacht to an unknown location, to which his fate is unknown. Fiona, now with a new job and a new position selling cups, sends a final voicemail to Jimmy's phone, saying: "Wherever you are....bye." Depressed from Mickey's marriage, Ian enlists for the army without the knowledge of his family, only telling a shocked Mickey. He leaves secretly the next morning, forging identification using Lip's ID, ending in a season cliffhanger.
