Unfit for trial

How do we categorize a defendant who was found unfit for trial because of mental illness, without mounting an insanity defense? Consider Shūmei Ōkawa who convinced a psychiatrist (and the judge) he was too nutty to stand trial at the International Military Tribunal for the Far East.
