Who was the overweight guy

Overweight guy (patient) who was singing opera. Prominent character. Also, it may be worthwhile mentioning that the film is in black and white, but with some color sequences during the hallucinations of patients.

Reading ... that was Larry Tucker. He was obsessed with the opera Pagliacci. No article for Larry Tucker.
