In short: There are several underlying problems here. There are too many issues for this proposed article to go to mainspace. I recommend dropping this particular title, and salvaging the content as discussed below. In detail: Firstly, as conceded in an earlier version of this draft article, is "there is no current standardization of care plan content across health and social care organizations, as each functions under their own methods for care plan creation." Problematically, this assertion lies behind a membership password portal, so this reviewer cannot attempt to remediate the content based on looking at that source. Second, the references point to these substantially divergent types of health "care plan", and synthetically abstract a proposed text that represents a list of basic dictionary terms. This "lowest common denominator" list of dictionary definitions is not well suited to an encyclopedia, absent any accompanying conceptual discussion of the underlying concept. Unless you have found sources that within themselves take a broad view of care plans, and advance further than dictionary definitions, as a reviewer I see no point in resubmitting this draft. Third, the proposed article is mistitled: the references and text include UK and Ontario sources. Fourth, it is clear that the extant Wikipedia articles, Health assessment, Medical case management, and Disease management (health) overlap appreciably with this proposal (and among themselves) at a conceptual level. As such, I would recommend salvaging this proposed text by carefully incorporating relevant content into one of these articles. Fifth, there are sources that come some way toward looking at this topic encyclopedically within narrower domains, for example, cancer survivorship e.g. see the editorial "Avoiding Implementation Errors in Cancer Survivorship Care Plan Effectiveness Studies." Deborah K. Mayer, Sarah A. Birken, and Ronald C. Chen. JCO Nov 1, 2015:3528-3530.

Draft:Plan of care (healthcare)[edit]

Hello, Beth.herlin: There are several articles within the general concept space of the proposed draft. What are your thoughts about using the present draft text within one of these extant articles, as a stepping stone to improving the suite of articles? I would be up for using the text that way if you green light the concept. If the content eventually grows to self-standing status, a new article could then be peeled off.
