- It mostly depend on how you map the Italian institution into your culture and therminology. Berlusconi and Napolitano are not the same as Fillon and Sarkozy. (France is "presidential", Italy is "parliamentar"). The executive power is held by Berlusconi, not Napolitano. And Berlusconi, in Europe, NATO, and whatever international organization meets with Sarkozy, not Fillon.

Being not a 1-1 relation, both the definition of "first lady" might be right.
