We require this information to verify that the image is legally usable on Wikipedia, and because most image licenses require giving credit to the image's creator.

It looks as though you pasted in sample content from a "how to write a biography" page. Please do not create articles in mainspace that aren't ready for the public to read. This page was apparently created by cutting and pasting some or all of the content of Draft:Richard D. Johnson, what we call a Cut and paste move. I've moved it back to draft space (with a prefix of Draft: before the article title). You can submit it for review using the Articles for Creation template on the page.
