That is the way the Oxford Dictionary of National Biography spells it and also the way it is spelled in the most important biography of Lettsom by J. John Coakley Lettsome → John Coakley Lettsom – The most common spelling of the name is Lettsom. Such is the spelling in the ODNB, in the referenced publications by the American Academy of Arts and Sciences, the Transactions & studies of the College of Physicians of Philadelphia, and the Postgrad Med J. The major biography by James J. Abraham is entitled Lettsom: His Life, Times, Friends and Descendants (1933). This is also the spelling of his name in his own publications. See Google Books search for "John Coakley Lettsom" and for "John Coakley Lettsome". His father was "Edward Lettsom". See also the inscriptions on the media at Wikimedia Commons.
